In this role, you’ll design and manage programs that address social determinants of health, enhance care coordination, and align with evidence-based public health practices. Your leadership and cross-sector collaboration will directly contribute to our vision of building a healthier, more resilient Douglas County. ESSENTIAL JOB RESPONSIBILITIES

  • Design, implement, and oversee innovative population health programs that focus on chronic disease management, preventive health, and the reduction of health disparities
  • Manage multiple projects simultaneously, ensuring alignment with Umpqua Health’s strategic goals, regulatory guidelines, and value-based care models
  • Collaborate with internal departments, healthcare providers, and community-based organizations to expand access to care and coordinate services for high-risk populations
  • Use data and analytics to identify trends, monitor program performance, and drive targeted interventions that improve health outcomes and reduce inequities
  • Lead initiatives aimed at addressing social determinants of health, including housing instability, food insecurity, and transportation barriers
  • Track and evaluate program outcomes, integrating insights and feedback to refine strategies and maximize community impact
  • Oversee budgets, staffing, and resources to ensure efficient, sustainable, and scalable program operations
  • Build strong partnerships with external stakeholders including public health agencies, academic institutions, and nonprofit organizations to support program growth and community outreach
  • Provide regular reports and presentations to leadership, funders, and community partners on program impact, progress, and strategic recommendations
  • Perform other duties as assigned to support Umpqua Health’s Vision, Mission, and Organizational Values

QUALIFICATIONSMinimum Qualifications

  • Bachelors or Masters, degree in public health, Healthcare Administration, or a closely related field (required)
  • Minimum of three (3) years of experience in population health, public health, or healthcare program management with a demonstrated understanding of community-based care initiatives
  • Proven track record of designing and managing programs that reduce health disparities and improve clinical and social outcomes
  • Experience using data platforms and analytics tools to evaluate program performance and implement quality improvement strategies
  • Strong project management skills with the ability to lead multiple concurrent initiatives while meeting deadlines and maintaining high standards
  • Excellent communication and interpersonal skills with the ability to engage diverse stakeholders and lead cross-functional teams
  • Knowledge of value-based care, social determinants of health, health equity frameworks, and public health policy
  • Experience managing program budgets and allocating resources in alignment with organizational priorities and funding requirements
  • Passion for health equity, community empowerment, and building systems that support better access and outcomes for all
  • No suspension/exclusion/debarment from participation in federal health care programs (eg. Medicare/Medicaid)
  • Proficient computer skills, including MS Office suite

Preferred Qualifications

  • Clinical experience or background.
  • Experience working with care coordination or case management teams in a managed care setting.
  • Experience considering the impacts of the work on multiple communities, including communities of color, in technical analysis.
  • Experience working on a diverse team with different communication styles.
  • Bi-lingual translation or translation capabilities a plus

PHYSICAL DEMANDS & WORK CONDITIONS

  • A typical office environment requires standing, sitting, walking, bending, and lifting up to 25 plus pounds.
  • Ability to travel and drive to in-person meetings or be on-site as required
  • May be exposed to patient and environment conditions such as unpleasant sights, smells and contagious diseases (clinic only)
