Patient Enrollment Lead
About the Role:
The Patient Enrollment Lead is a hybrid role—50% patient enrollment and 50% team leadership—responsible for driving high-quality patient engagement while coaching, developing, and supporting the enrollment team. You will monitor daily enrollment operations, ensure timely patient outreach, and maintain consistent processes across different programs. This role requires strong communication skills, exceptional attention to detail, and the ability to thrive in a fast-paced environment while leading others to do the same.
You will play a key role in identifying eligible patients, educating them on program benefits, and ensuring successful enrollment. You will also guide team performance, streamline workflows, and collaborate with cross-functional partners to elevate the overall patient experience.
You will:
Team Leadership & Development
- Oversee daily operations and metrics of the enrollment team by tracking call schedules, queue assignments, and other activities.
- Lead professional development initiatives by training and coaching team members, ensuring they have all the resources they need to be successful.
- Serve as a day-to-day resource for the team by answering questions, providing guidance, and managing all escalations for workflow issues.
- Monitor enrollment KPIs and daily referrals, ensuring timely outreach and consistent follow-through to drive patient enrollments.
- Utilize data and team insights to identify trends and process gaps, making operational refinements to strengthen throughput.
Patient Identification, Screening & Outreach
- Conduct patient outreach via phone, email, and text to explain program benefits, obtain consent, and move patients through the enrollment pipeline.
- Coordinate remote monitoring device shipments and collaborate with clinical, technical support, and care coordination teams to ensure seamless patient onboarding.
- Screen referrals to assess eligibility based on clinical criteria and chronic condition requirements.
- Maintain the status spreadsheet of patients identified and progress toward enrollment.
Enrollment Process Management & Optimization
- Address patient questions and provide clear, empathetic education about program expectations, remote monitoring, and chronic condition management services.
- Guide patients through required documentation/authorizations to complete enrollment, and ensure timely, accurate entry of all patient information.
- Identify themes in patient feedback and share recommendations to improve communication or marketing materials.
- Maintain regular communication with patients and internal teams to facilitate smooth enrollment and ongoing participation.

Qualifications:
- Education in an allied health professional field preferred.
- 3+ years of experience in healthcare, telehealth or a related industry.
- Demonstrated leadership experience through mentorship, training, or team coordination.
- Excellent verbal and written communication skills, particularly in virtual settings or over the phone.
- Proficient in using electronic health records (EHR) systems and other relevant software.
- Strong organizational, time management, and multitasking abilities.
- High level of accountability, reliability, and work ethic.
- Computer literacy in Microsoft Office, Google Suite, and ability to learn new systems quickly.
