More about the role:
Seeking an experienced and patient-focused LPN to support virtual patient inquiries, triage, and care escalations within the team’s telehealth platform. As part of the LPN team, you will be the first clinical point of contact for patients seeking support. You will assess concerns, provide guidance within scope of practice, and escalate cases appropriately to ensure safe, timely care.
Key Responsibilities:
- Respond to patient inquiries via phone, chat, and secure messaging
- Conduct structured triage assessments using established protocols
- Identify urgent or emergent cases and escalate appropriately
- Collaborate with RNs, NPs, and physicians to coordinate care
- Document all interactions accurately in the EMR
- Provide patient education and clear follow-up instructions
- Support quality assurance and continuous improvement initiatives
Key Requirements:
- Active, unrestricted Licensed Practical Nurse (LPN) license.
- Any previous experience in telehealth / digital health + using different software / platforms is a plus.
- Ideally, experience with weight management / obesity medicine / GLP-1 medications, and a genuine interest in the topic.
- Excellent communication and motivational skills, especially in a virtual care setting.
- Strong assessment and critical thinking skills
- Comfortable working in a fully remote, fast-paced environment & tech-savvy.
- Committed to continuous learning and patient-first care.
- Ability to work a 5 day week inclusive of one weekend day (i.e. Tuesday-Saturday or Sunday-Thursday)
