Abound Health is seeking an Intake Coordinator to join our team supporting individuals and families entering Innovations Waiver and State-Funded Services. In this role, you will report to the Intake Manager and serve as the first point of contact for clients and referral sources, ensuring a smooth, welcoming, and efficient intake experience while upholding Abound Health’s mission and values.

Key Responsibilities

  • Serve as the primary point of contact for referrals and intake inquiries within assigned counties
  • Collect, review, and coordinate intake, referral, and admission documentation
  • Communicate regularly with families, referral sources, and internal teams to expedite intake timelines
  • Coordinate with Program Directors and clinical teams to ensure smooth client transitions
  • Identify staffing needs and communicate with Staffing and Onboarding teams
  • Participate in intake-related meetings and advocate for clients and families
  • Ensure accurate documentation is uploaded and maintained in required systems

Qualifications

  • Credentialed as a Qualified Professional (QP) under one of the following:
  • Master’s degree in a related human services field with 1 year of IDD experience
  • Bachelor’s degree in a related human services field with 2 years of IDD experience
  • Bachelor’s degree in an unrelated field with 4 years of IDD experience
  • Strong attention to detail and organizational skills
  • Excellent written and verbal communication skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Collaborative, professional, and client-focused approach

Education

  • Bachelor’s or Master’s degree as required for Qualified Professional credentialing

Compensation & Benefits

This is a full-time, salaried, exempt position. Compensation is competitive and based on experience, education, and job-related skills. Abound Health offers a comprehensive benefits package, including health, dental, and wellness benefits beginning on the 60th day of employment, as well as a 401(k) plan available after 12 months of employment.
