- Schedule patients for visit plans at the beginning of each episode to ensure continuity of care.

- Process incoming referrals accurately and strive for admission within 24 hours of receipt.
- Document updates on patient care and communicate scheduling adjustments with team members.
- Manage patient visits across all disciplines according to care plans, frequency orders, and timelines.
- High School diploma or equivalent required.
- Strong attention to detail and accuracy in scheduling, reporting, and documentation.
- Excellent organizational skills to manage multiple tasks and ensure seamless scheduling processes.
- Effective communication skills, both verbal and written, to facilitate team collaboration.
- Problem-solving abilities to address scheduling challenges and operational issues.
- Experience in a home healthcare or clinical scheduling environment.
- Familiarity with EMR systems, particularly WellSky.
- Ability to adapt to changing schedules and priorities in a fast-paced setting.
- Previous experience working in a collaborative team environment.
