Healthcare Advisory Board Member - Aging, Complex Care & Population Health Strategy

We are seeking an experienced healthcare executive, geriatric care leader, or innovator in aging-focused healthcare delivery to join our Advisory Board and help shape the clinical, operational, and strategic direction of a forward-looking organization at the forefront of cognitive and complex care.


This role is intended for leaders with deep expertise in complex care management, value-based care transformation, population health, interdisciplinary care models, and scalable healthcare delivery systems. The ideal candidate understands how to align clinical excellence, operational discipline, reimbursement strategy, and long-term enterprise value creation in service of medically complex and aging populations.


As a Healthcare Advisory Board Member, you will provide strategic guidance on age-friendly care model design, frailty and risk stratification, complex chronic disease management, care transitions, post-acute and long-term care strategy, home- and community-based care models, and population health initiatives for high-need, high-cost populations. You will work closely with executive leadership to help ensure evidence-based, person-centered care models that improve outcomes, quality performance, patient experience, and total cost of care.

You will also advise on scalable service line development, integrated care delivery across the continuum, advanced care planning, interdisciplinary workforce design, regulatory and reimbursement strategy, and innovative programs addressing social determinants of health. Particular emphasis will be placed on building disciplined, data-informed operating models that support risk-bearing arrangements, payer-provider alignment, and sustainable growth.

This position is ideal for a healthcare leader passionate about advancing care for medically complex and aging populations by translating clinical and operational expertise into scalable, outcomes-driven strategies that improve longevity, independence, quality of life, and system-wide performance.

Responsibilities Include

Clinical Governance & Care Quality Oversight: Advise on excellence in clinical care delivery, patient safety, evidence-based practice, quality frameworks, risk management, documentation integrity, regulatory compliance, and performance measurement systems serving high-need populations.

Scalable Care Models & Integrated Delivery Strategy: Support the design of efficient, scalable healthcare delivery models, including interdisciplinary team structures, home- and community-based care, post-acute integration, and cross-continuum care coordination.

Value-Based Care, Risk Strategy & Population Health Alignment: Guide positioning within evolving reimbursement and risk-bearing environments, including value-based contracts, accountable care models, Medicare Advantage, bundled payments, and alternative payment arrangements.

Executive Collaboration & Organizational Alignment: Partner with executive leadership to align clinical strategy with operational, financial, and technology priorities while supporting provider engagement, change management, and interdisciplinary collaboration.
