You will engage directly with patients in homes, hospitals, and community environments to assess needs, build trust, and develop personalized care plans that address both medical and social determinants of health. The position focuses on removing barriers to care by connecting individuals with essential services such as housing, food assistance, transportation, and behavioral health support. You will collaborate closely with interdisciplinary care teams to ensure coordinated, patient-centered outcomes. Accountabilities
- Initiate direct outreach to eligible patients in hospitals, clinics, homes, shelters, and community settings to introduce services, complete screenings, and establish engagement in care management programs.
- Develop, implement, and monitor individualized care plans in collaboration with patients, families, and interdisciplinary care teams, ensuring progress toward health and wellness goals.
- Provide ongoing education to patients using culturally and linguistically appropriate methods to support disease self-management and health literacy.
- Coordinate with healthcare providers, social workers, and community organizations to connect patients with essential resources and services.
- Assist patients in navigating healthcare systems, including scheduling appointments, attending visits when needed, and supporting medication access and follow-through.
- Maintain accurate and timely documentation of patient interactions, care plans, and progress updates using care management systems such as Epic.
- Advocate for patients by identifying barriers to care and facilitating access to housing, food assistance, financial support, behavioral health services, and other community resources.
- Participate in case reviews, team meetings, training sessions, and community outreach activities to support program goals and continuous improvement.
- High school diploma with relevant community experience or a Bachelor’s degree in a related field.
- Minimum of 2 years of experience in healthcare, public health, social services, or community-based outreach work.
- Valid driver’s license required for community-based fieldwork.
- Strong understanding of healthcare systems and social determinants of health.
- Excellent interpersonal and communication skills with the ability to build trust across diverse populations.
- Strong organizational and multitasking abilities, with the capacity to work independently and within a team.
- Proficiency in Microsoft Office tools and experience with data entry and tracking systems.
- Cultural competence and awareness of how language, socioeconomic factors, and culture influence health outcomes.
- Fluency in Haitian Creole or Spanish is strongly preferred.
