Role & Responsibilities
- Lead design, implementation and day-to-day delivery of clinical programs (care management, transitions of care, utilization management) for US client accounts in a remote model.
- Manage, mentor and operationalize clinical teams and vendors; define KPIs, workflows and expectations to drive consistent outcomes.
- Partner with product, analytics, implementation and client stakeholders to translate clinical requirements into platform configurations, care pathways, and SOPs.
- Monitor clinical quality and performance metrics (HEDIS, CMS measures, readmission rates); surface gaps and lead continuous improvement initiatives.
- Oversee EHR and telehealth integrations, ensuring documentation standards, interoperability and smooth data flows between clinical systems and analytics platforms.
- Maintain regulatory and compliance oversight (HIPAA), develop clinical policies, manage risk escalations and ensure audit readiness.
Must‑Have
- Active clinical credential (e.g., RN, NP, PA) or equivalent clinical background with demonstrated leadership in clinical operations or care management.
- Proven experience operationalizing care management or population health programs and leading cross-functional clinical teams.
- Practical knowledge of Electronic Health Records and integration points with analytics platforms (experience with Epic strongly preferred).
- Solid understanding of quality measurement frameworks and regulatory requirements (HEDIS, CMS measures, HIPAA).
- Experience working with healthcare analytics or SaaS platforms and in client-facing implementation roles.
- Formal training in project management or process improvement (PMP, Lean, Six Sigma) or equivalent experience.
