The Company

Expert Radiology is a high-growth (100% growth each of the past 4 years, accelerating this year) teleradiology platform specializing in outpatient MRI radiology. We produce the best radiology reports on the planet. We serve 350+ imaging facilities across 28 states, are licensed in all 50, and will continue to grow rapidly. Though we are a seasoned practice in many ways, We are building a technology-enabled platform — proprietary AI to assist radiologists in spine MRI interpretation and illustration, a structured data flywheel, and a client and client/patient/doctor/attorney-facing workflow layer.

This is not a mature business looking for a caretaker. This is a founder-led company scaling aggressively over the next 12–18 months and beyond.


The Role

You will be the operational counterweight to a physician-CEO whose strengths are vision, product, and clinical authority. You own everything that turns strategy into results: operational execution, team leadership, financial rigor, process design, and capacity management. Specific mandates include:

  • Build and own the operating infrastructure for a company in a period of rapid scaling
  • Design and implement scalable MRI workflows with at least 2 materially different margin profiles — the cost engineering challenge is real and central
  • Manage radiologist capacity planning, recruiting pipelines, and utilization to stay ahead of volume growth
  • Partner with the CEO on strategic growth initiatives including potential acquisitions in a fragmented market
  • Own the weekly scorecard, operating cadence, and accountability rhythms (we run EOS)
  • Lead a growing team across operations, client services, and technology — current headcount ~37, scaling significantly within 18 months
  • Implement HIPAA-compliant agentic AI company-wide to scale the business without commensurate increase in headcount.


Why This Role Is Different

Most COO seats at this stage are maintenance jobs. This one is a build. You are constructing the operating system for a platform company with defensible AI and a proprietary data asset — not managing the status quo. The CEO is radically transparent (Culture Index and OMG assessments are shared openly, including his own), moves fast, and will give you real authority. The company is based in Puerto Rico under Act 60, which means 0–4% tax on equity gains at exit if you are willing to move to PR.


You Are Likely a Fit If

  • You left consulting for an operating role and have 3+ years running operations at a PE-backed or founder-led healthcare services company
  • You have built teams and processes during a period of rapid scaling (50%+ annual growth), not just managed existing ones
  • You are high on follow-through, detail orientation, and process discipline — and comfortable partnering with a visionary founder who though very capable of follow-through is less naturally-inclined to enjoy it
  • You understand healthcare services economics: credentialing, multi-state licensing, utilization management, provider recruitment
  • You are energized by ambiguity and comfortable with a company where systems are being built for the first time


Compensation

Base salary: $200–300K depending on experience. Potential equity (details in conversation). Performance bonuses tied to company milestones. Act 60 tax advantages for candidates willing to move to PR.
