Overview:
We are seeking a Care Coordinator to support case management teams in a healthcare/managed care environment. This role focuses on coordinating care services, managing member interactions, and handling administrative support to ensure timely and effective care delivery.
Key Responsibilities:
- Support case management processes and coordinate member care activities
- Request and track clinical information from providers
- Schedule appointments, screenings, and transportation for members
- Educate members and caregivers on care plans, benefits, and follow-up steps
- Monitor member needs and ensure timely follow-ups
- Act as a point of contact for providers, members, and internal teams
- Assist with outreach campaigns and special projects
- Maintain documentation and perform general administrative tasks
- Coordinate schedules and logistics for healthcare staff (e.g., Nurse Practitioners)
Required Qualifications:
- High School Diploma required; Associate degree or healthcare training preferred
- 3–5 years of experience in a professional or healthcare environment
- Strong organizational and multitasking skills
- Excellent communication and customer service skills
- Proficiency in Microsoft Office (Word, Excel, Outlook)
- Ability to handle multiple priorities in a fast-paced environment
Preferred Qualifications:
- Experience in managed care or healthcare coordination
- Familiarity with case management systems or healthcare tools
- Background in medical assistant, LPN, or healthcare support roles
